At Rooted Health Collective, we believe care should be simple, affordable, and transparent. Below you’ll find our current flat-rate fees and at-cost services. There are no hidden charges, just straightforward, compassionate care. 

 $40 per visit
Up to 45 minutes for medical, mental health, or urgent care appointments.
Includes time for discussion, assessment, and follow-up planning. 

 At-cost pricing (usually $20–$30 per test)
Includes common labs such as CBC, CMP, and A1c.
Rooted Health Collective partners with multiple accredited laboratories to ensure that patients across Florida have access to the most affordable, high-quality diagnostic testing available, regardless of location.
We do not add markups; you pay only the lab’s direct rate. 

 Included with your visit
Covers medication counseling, e-prescribing, and refills as appropriate. 

Forms & Letters Fees

Work or School Excuse Letter: $15

Simple documentation for work, school, or other routine needs.

Basic Form Completion (1–2 pages): $25

Includes forms requiring limited review and provider completion.

Extended Form Completion (3–5 pages): $50

Includes more detailed forms requiring additional review and documentation.

Complex Forms (FMLA, Disability, Leave of Absence, Accommodation Requests): $75

Includes comprehensive review of the medical record, clinical assessment, and detailed documentation.

Expedited/Same-Day Processing (when available): Additional $25

Available for urgent requests at the provider's discretion.

Medical Records Requests:

Electronic records provided at no charge. Fees for paper copies or extensive record requests may apply as permitted by state and federal regulations.

Forms & Letters Policy

Form and letter completion fees are not covered by insurance and must be paid before processing. Requests are typically completed within 3–5 business days. Completion of a form, letter, or evaluation does not guarantee approval, certification, accommodation, or issuance of documentation. All recommendations and supporting documents are based on clinical findings, medical necessity, and professional judgment.Covers documentation for work, school, disability, or other needs.
Please allow 24–48 hours for completion of most requests.
Exceptions: Sick notes for work or school are typically completed the same day of the visit to minimize disruption.
